Hair Transplant

A hair transplant surgery is a surgical technique performed to replace existing androgenic hair on the scalp with new androgenic hair. Although widely referred to as a "Hair Transplant," its official name is actually "Follicular Unit Transplant" due to the fact that individual follicular units are transplanted rather than complete hairs.

The main concept behind this surgery is that an individual's hair loss has a genetic cause and that the number of hairs on the scalp is predetermined at birth. Hair follicles, which are genetically programmed to produce hair, exist in particular patterns on the human scalp. A surgeon can remove small groups of these follicles known as "follicular units" and transplant them into areas of the scalp where hair has thinned.

A skilled surgeon can extract follicular units one at a time with tiny punches that are approximately 0.6-1 mm in diameter. These punch sizes help to ensure the survival of each individual graft because there is no trauma or damage done to surrounding blood vessels, which would have occurred if larger punches were used.

In order to create a natural look, the surgeon will transplant follicular units in their naturally occurring patterns. This way a patient's hair loss or baldness is not as apparent after surgery compared with traditional methods, such as strip surgery where a band of skin containing multiple hairs is removed from the scalp and cut into smaller pieces.

In addition to follicular unit transplantation, robotic hair restoration is a newer technique in which an automated robot using stereoscopic cameras with magnifying lenses performs the procedure. This has been used for both conventional and robotic follicular unit extraction procedures, but no comparative studies have been conducted yet showing that one procedure is superior to the other.

Historical Timeline of Hair Transplant

It is unclear when humans began to use hair transplantation, but there are some records of people surgically altering their appearance in the late 1800s. In 1868, during the American Civil War, a New York physician named J. Marion Sims used his newly developed technique for repairing severe facial injuries with flap surgery in order to help wounded soldiers. In 1889 he performed hair transplant surgery on patients who had lost their scalp and facial hair after suffering injuries in a train crash.

In 1912, Dr. Richard Ardoin of New Orleans began performing the first recorded modern hair transplants in his practice. He developed a technique where multiple strips of skin containing hairs were moved from the back of the scalp to areas where hair was missing. However, because the donor site often became much more noticeable than before surgery it did not become very popular.

In 1952, U.S. plastic surgeon Alan Dunn developed a new technique for transplanting individual follicular units in order to create a more natural look than what was previously possible with strip surgery. His work set the standard for hair restoration and led to a boom in procedures all over North America and Europe.

In the late 1950s, the Canadian-born physician and surgeon Norman Orentreich began performing hair transplants at his practice in New York. He believed that transplanting body hair from different sites on the body could help to halt hair loss and promote growth. In 1959, he performed an experimental hair transplant on a woman who had lost all of her scalp and facial hair as a result of a rare disease called Alopecia universalis. The procedure was successful and due to its uniqueness it received national media attention. Orentreich continued his work and in 1968 he published the first paper on hair transplantation.

Since then, several surgical methods have been developed as discussed above.
