Hair Transplant Methodology
This is an interesting subject, with advances occurring on a regular basis. The most notable of these improvements in the past half-decade has been the introduction of Follicular Unit Transplantation (FUT). This technique differs from many traditional procedures in that it allows for preservation of donor hair to nearly 100% . By doing so, physicians have been able to offer patients another option in hair transplantation surgery, that being the immediate or recipient area/scar concealment.
This methodology involves six main steps:
The donor strip is removed from the posterior scalp under local anesthesia utilizing a scalpel blade . The location of this incision varies with the individual surgeon however it is generally placed within 1cm of the proposed recipient area.
The strip is dissected free from fat and other tissues so as to yield individual grafts known as follicular units . A small circle of skin around each unit is left intact which serves to protect the graft during preparation and transportation back to the recipient site. After this has been completed, the donor strip tissue is sutured closed with absorbable suture material.
The grafts are immediately harvested and prepared for placement in the recipient area . This process involves separating each follicular unit from the others and trimming away excess tissue such as sebaceous glands and hair shafts. A small hole is made in the middle of the head exactly where the surgeon wants the unit to be placed. The graft(s) is then carefully inserted into this hole and secured in place with a few simple stitches. The skin surrounding this opening is then sutured together with either finger-tied or Prolene suture material, laser cut to minimize scarring.
The patient will go home after a short recovery period with a small dressing covering the graft insertion point. A water soluble bandage or simple gauze may be used in place of medical tape depending on the individual procedure. Most patients will require no more than an over-the-counter pain medication to control any post-operative discomfort during this early recovery period.
The patient will need to return to the surgeon's office in approximately 7-10 days for a follow up visit at which time the dressing will be removed. At this point, any early scabs from the donor strip extraction should have fallen out already and the grafts may be lightly rinsed with saline to remove any residual crusting.
Patients are seen again at 1 month post-op and 3 months post-op for further follow up care.
This methodology has several advantages:
The recipient area is usually quite close to the original hairline and as such, does not create a harsh transition between existing hair and implanted results.
Patients can be seen immediately after surgery by their own physicians for follow up care.
Since the donor strip is removed intact, there is little to no post-op morbidity. The patient may feel some mild discomfort which is readily treated with over-the-counter pain medications. There are no stitches to remove and patients return immediately to their normal daily activities within only a few days.
Strip harvesting maintains the natural directionality of hair growth in the donor area . This allows for increased graft survival and is therefore associated with higher success rates.
Since the transplanted grafts are immediately adjacent to existing native hairs, surgery is not immediately obvious to others. The transplanted grafts blend naturally into the scalp since they are located alongside existing hairs.
